Why your MRI is a picture of your age, not your pain
When we receive an MRI report mentioning words like “herniated disc”, “dehydration”, or “protrusion”, it is completely natural to be scared. The immediate tendency is to think that our spine is “broken”, that we are fragile, and that we must stop moving to avoid making things worse. However, modern science tells us otherwise: your MRI shows your age, not your pain.
What the data says: Prevalence in asymptomatic people
A landmark study published by Varun S. et al. (2025) analyzed thousands of people who had absolutely no back pain. The results are eye-opening:
- 30% of the healthy, pain-free population presents lumbar disc herniations.
- Segmented by age, the presence of herniations is 35% in individuals aged 40 or older, compared to 25% in those under 40.
- Other findings like disc dehydration (normal wear and tear) appear in more than 50% of healthy 40-year-olds and over 80% of those aged 50.
What does this mean? It means that having a herniated disc or spinal wear is the exact equivalent of having wrinkles on your skin or gray hair. They are normal signs of the passage of time that do not determine whether we will suffer pain.
The danger of kinesiofobia
The real problem with focusing excessively on a radiological image is that it generates kinesiofobia (fear of movement). Believing our spine is fragile leads us to avoid movement, stiffen up, and drop the activities we love.
Ironically, avoiding movement weakens protective muscles, reduces circulation to the disc (which needs movement to receive nutrients), and increases the sensitivity of our nervous system. In short: fear of damage is often far more disabling than actual tissue damage.
Conclusion: Your spine is robust and resilient
An MRI is an excellent tool to rule out serious pathology, but it is not a picture of your pain. Your spine is an extremely strong, robust structure designed for movement.
If you have a herniation and back pain, the path to recovery does not lie in absolute rest or the fear of bending, but in active coping: reintroducing graded movement and teaching your brain that your body is still safe and capable.
